​Instructions for a Virginia Police Clearance
If you need a notarized police clearance letter for your international adoption dossier, you will need to obtain an additional clearance. You must complete form S.P. 167, entitled Criminal History Record Name Search Request. 

Complete as follows:

•  PURPOSE OF THIS REQUEST: Check “Adoption-International” and specify the country

•  NAME INFORMATION TO BE SEARCHED: Complete legibly and fully

•  AFFIDAVIT FOR RELEASE OF INFORMATION: Notarized signature of applicant is needed

•  SIGNATURE OF PERSON MAKING REQUEST: Leave blank!!!

•  NAME AND MAILING ADDRESS: Cite full name and address of applicant

•   FEES FOR SERVICE: Check “$15.00 Criminal History Search”

•  METHOD OF PAYMENT: Use Either a Certified Check, Money Order, or Visa Credit Card or MasterCard Credit Card.  If you use a Credit Card, be sure to provide the card number, expiration date, and full name of the card holder.  Personal checks are not accepted.

Print one (1) copy of the completed form for each applicant, sign before a Virginia notary public, and send the form to:
